Fall 2010 Course Registration Form

Students wishing to be full-time must register for at least 12 credit hours. Registration for Campus and Online Courses is now closed. This form may only be used to register for onsite courses(Distance Education).

Overview: Note which of the following parts of the form you need to complete.

Part A: General Information (All Students must complete this section)

1. If you were a student in Winter or Summer 2010 please proceed to Number 2. If not, one of the following will apply to you.

New Students: please ensure you have completed the Regular Application Form or you have completed the online Occasional Student Application Form.
Former students returning to continue their studies after one or more semesters away from MCS may need to complete a Re-Admission Application Form or a Re-Enrollment Form.
Occasional Student who has completed or will complete five courses prior to the start of the Fall semester will need to complete a Regular Application Form.

2. Mature Adults Only: Are you 55 years old or older by September 30, 2010? Yes No If yes, please enter your date of birth:(DD/MM/YYYY) . You are eligible for a Mature Adult discount (See the Fall 2010 Registration Procedures for more information). If you have not previously done so, please fax(416-482-7004) or mail a copy of a government-issued photo identification card so that your tuition reduction can be processed.

3. Payment Options: Indicate which of the following Payment Options you will be using to pay for your courses. Additional details regarding the Payment Options was explained on the Fall 2010 Registration Procedures page:

On or before August 19, 2010 I will arrange with the Finance Office to pay for my fees in full by cash, cheque, debit card or credit card.
On or before August 19, 2010 I will arrange with the Finance Office to use Deferred Payment Plan.
On or before August 19, 2010 I will arrange interim financing by contacting , since I have not received my government-issued Student Loan.
