Winter 2010 Registration Form

Part A:  General Information (All Students must complete this section)

1. New Students: please ensure you have completed the Regular Application Form or you have completed the online Occasional Student Application Form.
2. Former students returning to continue their studies after one or more semesters away from MCS may need to complete a Re-Admission Application Form or a Re-Enrollment Form.
3. Occasional Student who has completed or will complete five courses prior to the start of the Fall semester will need to complete a Regular Application Form.
   

2. Mature Adults Only: Are you 55 years old or older? Yes No If yes, please enter your date of birth:(DD/MM/YYYY) . You are eligible for a Mature Adult discount (See the Winter 2010 Registration Procedures for more information). If you have not previously done so, please fax(416-482-7004) or mail a copy of a government-issued photo identification card so that your tuition reduction can be processed.

3. Payment Options: Indicate which of the following Payment Options you will be using to pay for your courses. Additional details regarding the Payment Options was explained on the Winter 2010 Registration Procedures page:

On or before December 10, 2009 I will arrange with the Finance Office to pay for my fees in full by cash, cheque, debit card or credit card.
On or before December 10, 2009 I will arrange with the Finance Office to use Deferred Payment Plan. 
On or before December 10, 2009 I will arrange interim financing by contacting , since I have not received my government-issued Student Loan.
