Completed forms can be sent to the school by mail or fax, or they can be scanned and emailed to the Registrar’s Office at If a payment is required, we recommend that you either send it by mail so you can enclose the payment or fax it and phone the Finance Office (1-800-295-6368) with your Visa or MasterCard information. A form will not be processed until the payment is received.
